Admission of Service
- The resident / carer will be admitted to the service
after signing the "Individual Plan" once being
assessed as suitable for admission to the service by
the multi-disciplinary professional team.
Termination of Service:
If a resident encounters any of the following situations,
the case manager would discuss the service termination
arrangement with the resident/carer/hostel staff:

The resident has moved away from the hostel and has not transferred to
any private residential care homes for persons with disabilities in Hong Kong
Island and Kowloon regions.

The resident has been hospitalized for over 3 months and does not have
an discharge plan.

The resident has completed the "Individual Plan" and has agreed of not
continuing or starting a new plan.
The resident has already passed away.

The resident or carer voluntarily requests for the
withdrawal of service.

The procedures or conditions during service provision have
caused physical or emotional harm to our staff, and no consensus
can be made after mediation.

The resident failed to cooperate with the service arrangement
and no consensus can be made after mediation.
