Requirements:

• Degree holder in Social Work and registered under Social Workers Registration Ordinance;

• English Language (Syllabus B) and Chinese Language at Grade E or above in HKCEE / Level 2 in HKDSE;

• 5 years' relevant post-qualification experience is preferable;

• Proven track record in managing elderly funding projects and with experience in dementia services is highly preferable;

• Strong leadership and people management skills with excellent presentation, collaboration, networking and organizational abilities.

Job Duties:

• Manage, implement and monitor the “JC Brain Health: Jockey Club Dementia Screening and Community Support Project”;

• Coordinate project team for service provision and data collection and entry of assessments needed for intake and care planning;

• Assist medical practitioners in diagnosis and clinical follow-up;

• Conduct case management for each case, assessing the condition of care recipients and family caregivers, and designing appropriate care plans;

• Design discharge plan, discharge output and long-term care plan for each case;

• Coordinate public education talks or other promotional activities;

• Co-work with clients and academics in progress and outcome documentation and evaluation.

Working Hours: 44 hours per week

Working Location: Kwun Tong
