Requirements:

• Holder of a Postgraduate Diploma or Bachelor Degree in Dietetics, Nutrition or related discipline, with at least 3 years of post-qualification relevant working experience;

• Registered Dietitian with valid professional credentials;

• Holder of Accredited Dietitian (AD) credential under the Hong Kong Academy of Accredited Dietitians (HKAAD) or eligible for accreditation is highly preferred;

• Possess the Child Protection Online Training (Module 1 & 2) e-certificate;

• Experience in primary healthcare, chronic disease management, elderly care, community health or health promotion services will be an advantage;

• Good command of spoken and written English and Chinese (including Cantonese and Putonghua);

• Good communication, presentation and interpersonal skills;

• Self-motivated, proactive and customer-oriented, with the ability to work effectively within a multidisciplinary team.

Job Duties:

• Provide professional nutrition assessment, dietary counselling and individualized nutrition care plans for clients with chronic diseases, nutritional risks and other health conditions;

• Deliver nutrition assessment, counselling and follow-up interventions for clients enrolled in the Chronic Disease Co-Care Pilot Scheme (CDCC) and referred from Nurse Clinics and primary healthcare services;

• Collaborate with nurses, doctors and allied health professionals in multidisciplinary case management and care planning;

• Support the implementation of the WHO Integrated Care for Older People (ICOPE) programme, including nutrition screening, comprehensive assessment, care planning, health education, follow-up consultations and evidence-based interventions for older adults;

• Provide outreach nutrition education, health talks, cooking demonstrations, workshops and training programmes for community members, caregivers, schools, corporate partners and healthcare professionals;

• Develop nutrition education resources, maintain accurate service records and statistics, and support programme evaluation, reporting and documentation requirements;

• Perform any other duties as assigned by supervisors.

Working Hours: 22 hours per week (Half-time) / Flexible working arrangement for Part-time appointment

Working Location: Kwun Tong / Kowloon City
