Requirements:

• A Certificate of Registration (General) with valid Practising Certificate issued by the Nursing Council of Hong Kong;

• English Language (Syllabus B) and Chinese Language at Grade E or above in HKCEE/ Level 2 in HKDSE;

• Possess the Child Protection Online Training (Module 1 & 2) e-certificate;

• Minimum 2 years of clinical experience, with supervisory and project management experience preferred;

• Experience in elderly care, community healthcare or integrated care services is a must;

• Knowledge of research methodologies, health informatics and quality management systems is preferable;

• Demonstrated digital literacy and data management capability, with experience in using client information systems, case management platforms, or healthcare databases will be an advantage;

• Strong leadership, communication, stakeholder engagement, and report-writing skills.

Job Duties:

• Lead the planning, implementation, monitoring, and evaluation of a Project supporting older people to develop and/or maintain intrinsic capability;

• Supervise project staff and coordinate multidisciplinary teams across elderly service units;

• Provide clinical oversight for screening, assessment, intervention, and care management processes to ensure compliance with WHO ICOPE protocols and evidence-based practices;

• Monitor the quality and consistency of screenings and assessments conducted by trained volunteers and other social and health care professionals;

• Act as the primary contact person with the research team at a local university to coordinate research implementation and ensure adherence to approved research methodology and study protocols;

• Monitor data quality, completeness, and accuracy within the Care Management Platform;

• Maintain effective communication with participating elderly service units and community healthcare providers, and facilitate collaboration among multidisciplinary service providers;

• Facilitate knowledge exchange between practitioners and research partners;

• Perform other related duties as assigned by supervisors.

Working Hours: 44 hours gross per week

Working Location: Kwun Tong District with outreach community work required
