Requirements:

Degree holder in Social Science, Counseling, or a relevant discipline; substantial relevant post-qualification experience, at least 4 years of which in managerial level;

English language (Syllabus B), Chinese Language at Grade E or above in HKCEE or Level 2 in HKDSE;

Relevant experience and training in case management, risk management, or performance improvement is highly desirable;

Proven ability to collaborate effectively with individuals to identify problems, explore innovative solutions, and evaluate the effectiveness of implemented strategies;
-
Strong analytical skills with good presentation and writing abilities;
-
Exceptional communication and interpersonal skills, with a focus on teamwork and stakeholder engagement.
Job Duties:

Initiate, plan, and oversee service quality assurance and improvement initiatives for elderly frail case teams;

Review and analyze case and risk management practices, providing expert consultation to the teams;

Develop and implement robust case and risk management protocols;
-
Coordinate the implementation of performance and quality improvement initiatives within frail case teams;
-
Facilitate the sharing of knowledge, experiences, and best practices among service units to enhance service delivery;

Organize and coordinate relevant staff training programs to ensure effective implementation of quality improvement strategies;

Plan and write service proposal for applying for external funding.
