Requirements:

• A University Degree in Social Work and registered under Social Workers Registration Ordinance;

• English Language (Syllabus B) and Chinese Language at Grade E or above in HKCEE/Level 2 in HKDSE;

• 2 years of relevant post-qualification experience is preferable.

Job Duties:

• To plan, develop and execute the Project for hospital discharge and outreach services for elderly with limited social support;

• To work collaboratively with different teams and professionals to ensure effective service delivery;

• Solid experience in working in hospital is preferred;

• Excellent interpersonal and communication skills, with a positive attitude and demonstrate proactiveness in achieving the best standards of care and collaboration.

Working Hours: 44 hours per week, 5-Day work

Working Location: Kwun Tong and Tseung Kwan O District
